Common instructor mistakes

Classroom management assists with identifying those who may not be understanding the task. Once the student is identified you can quietly ask if they need additional help and or clarification.

Anytime an instructor thinks a student is struggling, the instructor should approach the student and offer assistance.

I don't necessarily agree. I try to put all students at ease and let them know if they have a question, probably many others in the room may have the same question and let them know not to be afraid or embarrased to stop me and ask for clarification.
