This session helped reiterate the importance of proper note-taking. Some possible reasons that may come up were listed, like: continuity of service, accountability, QA, legal & resource allocation.

The session also discussed some professional & guiding principles for documentation, such as: custodian ID, that notes are clear, time-stamped, legible, permanent ink (blue or black), uses acceptable terminology, is current, accurate, chronological, appropriate & complete.

Additional considerations for note-taking discussed included: being objective vs subjective, proper ways of handling documentation errors & the proper use of blank space.

The MAPP (Motive, Assessment, Progress & Plan) model was also presented for Career Service Professionals who document student/graduate interactions.

Lastly, if all else fails with employment verification, an option to seek out employers to complete & sign a Job Description form is presented as well as some recommendations on how to proceed with it.
