No question that everyone on the staff and faculty have to have the same vision. Do your admissions people participate in the orientation to affirm their understanding of the program?
Is the end of term feedback done face-to-face with an individual student meeting with the department head and his/her instructors? Sounds pretty intimidating. I'm not sure I would be willing to provide negative feedback in that situation.

Over 90% of colleges in the traditional sector have some sort of transition course for at least some of their students. Usually the courses include the topics you have identified. Are these skills being taught in an actual course? If so, is it for credit and part of the curriculum? All the skills are also useful when the student enters the workforce.

Great practice. Are these meetings required for all instructors, including adjuncts? How often do you have them and who runs them? Do your colleagues participate enthusiastically or are they indifferent or hostile to the subject?

Sounds like this system works well for you now, but what about the future after the school grows? Do you have a plan to accommodate a larger student body?

Your survey questions require students to be thoughtful and introspective. Have you been successful in getting them to do that? If so, how? Most of us have been frustrated by their proactive participation. You seem to have a different application of secret shoppers. I'm most familiar with the concept in the admissions process. You seem to be suggesting that you have "shoppers" actually assume the role of enrolled students. Is that correct and how do you pull it off?
