How often do you have students voluntarily re-take a course because they feel weak in the subject? How does that effect their progress through the curriculum? Is this in addition to their course load or instead of a course?
That's pretty scary. Is this because everyone is oblivious to the issue or they don't share what's going on in different departments?
It sounds like the students are already expanding your current survey to include institutional issues. Are you able to share these results with the appropriate people who can address the issues? Are the surveys part of the instructor's evaluation? If so, what's the impact of non-related negative responses?
How often do students use the suggestion/comment procedure? What issues do they address? Who's responsible for responding to the entries?

Have you studied the experience of other schools that have student councils? Do you settled on a framework and by-laws? How will representatives be chosen?

Are the teaching assistance programs for students with low GPAs? Are these done after class? Is the tutoring done by faculty or advanced students?
Cynthia, do you find those daily contacts provide better information about the student's attitude than the surveys?
