Hi Betsy,

ACCSC has a Monograph Series publication on Institutional Assessment and Improvement Planning on thier website here http://www.accsc.org/UploadedDocuments/2014%20Monographs/Institutional%20Assessment%20and%20Planning%20Monograph%202014.pdf that we have used in the past as a base table of contents - then we built in some additional, campus specific categories.  Hope that Helps.
